Delete all preset files from the game folder, start the game again and create a new preset. That preset will have all values reset to the default.
There should be an option that says FFXIV_HUD_TOP/BOTTOM in the Reshade effects tab. I forgot the exact name of it. Enable both and the options blurring will go away. Just FYI, some presets are not made for gameplay, only for screenshots.
